2. Discipline Specific (Needs to be renewed every 3 years)
   
   Choose your discipline:
• Biomedical Research Investigators
• Social & Behavioral Research Investigators
• IRB Members
• Research with Data or Laboratory Specimens-ONLY
• Students Conducting No More than Minimal Risk
  -OR-  Students – Class Projects

Be certain to include CITI certificates when submitting your proposal to irb@lewisu.edu.
